Missed Appointments

If you are late by 10 minutes or more, it will be considered a missed appointment and we will need to reschedule.

If two appointments have been missed or cancelled last minutes, a missed appointment fee of $60.0 will be due before we rebook.

Insurance and Finances

We cannot direct bill with certain insurance companies.

We cannot direct bill with private insurance when it coordinates with extraction.

Please inform administration of all insurance policies.

We bill with social programs such as LIHB, DASP, NIHB, and CDCP.

Deposits are required before proceeding with treatment that will not be completed in a single day.

Professional Fees are non-refundable.
